VCAM Deliverables Checklist
Use this checklist to keep track of your forms. Upon selection, you will be called upon to provide a signed form for every individual, location, music track or IP that is shown in your video.

Video Releases
- These are for the people and places that appear on-camera.

Appearance Release
Everybody who worked on, or appears in, your submission has to sign this. -
Clearance Release
This matches the personal releases up with your video. Tedious, but necessary. -
Location Release
You need one of these if you shot your submission on private property.
- Music Releases
- Music is complicated! You need both of these for every track you use:

Master Use & Sync Release
This release gets signed by the person who owns the rights to the master recording of the track you are using, and/or by the person who owns the copyright to the music itself. -
Music Cue Sheet
Use this form if you included any APM music in your video.
- Video Submission Release

Materials Release
Any copyright-owned or personally created IP (intellectual property) that is not prominently featured in a production (e.g. artwork, posters, sculptures, personal photographs, etc. appearing in the background) must be released by signing this document. -
Video Submission Clearance Report
Use this form to list all of the individuals who appear in and worked on your video, so we can match their release forms to your submission.
