If we acquire your VCAM, we will ask you to send us:
-Split audio tracks or OMF export
-Full resolution video file
-Release forms: Appearance, Crew, Location, Music
-APM Music: Track information (no release form necessary)

Kris, you only need one release form. Be sure to indicate on the form that he served in both capacities.
