I am not particularly worried about a nuclear missile being launched and reaching its target. I would be a little concerned about the retaliatory response. Am I to assume if a launched nuclear warhead is fired and not successful in targeting, the perpetrator is sent a happy meal coupon with a 30 day expiration date.

I am also vexed by nuclear destination. You fire a "nuclear" warhead. How does the international community establish it's intended target?
