Ajax were dumped out of the KNVB Beker by struggling FC Volendam thanks to Jack Tuyp's extra-time goal.
Eredivisie basement boys Volendam can hardly buy a win in the league having managed just four points in 10 games, but saw off the high-flying Amsterdam giants thanks to Tuyp's 111th-minute strike.
Eredivisie champions PSV Eindhoven were also ousted in extra time, losing 1-0 at league leaders AZ Alkmaar.
PSV last won the competition in 2005 and they will have to wait another year as David Mendes da Silva's goal in the 98th minute was enough to send AZ through.
Vitesse Arnhem were the only other Eredivisie team to exit following a 1-0 defeat at FC Lienden, Erik Kruik scoring the only goal of the game 10 minutes from time.
The five other top-flight teams facing lower-league opposition all progressed, with Heerenveen doing so in style courtesy of a 7-0 win over Haaglandia/Winston.
The hosts were 5-0 up inside 33 minutes, with Danijel Pranjic grabbing a hat-trick in that time.
Two goals from Hursut Meric helped ADO Den Haag to a 4-1 win over Fortuna Sittard, while FC Twente were 3-2 winners at FC Eindhoven.
