Breaking the silence on HIV in Egypt
One HIV-positive Egyptian speaks out against stigma surrounding the virus 30 years after the first case was diagnosed.
Thirty-one-year-old Magid is tired of being silent.
Diagnosed three years ago with HIV, the virus that causes AIDS, Magid, a resident of Alexandria, Egypt, kept his health status a secret from most, opting to tell only a handful of friends and family members weeks after he discovered he had been infected.
But he chose June of 2011 as the month in which he would shed his relative anonymity in Egypt and speak out as a voice for people living with the virus.
"Last week, I broke the silence by becoming the first Egyptian to speak out about AIDS," he tells Al Jazeera, days after his appearance at a press conference organised by the Forum to Fight Stigma and Discrimination against People Living with HIV/AIDS in Egypt.
"I was scared. But Egypt [was] in urgent need for someone to do that, because people with HIV are suffering from stigma and discrimination. Policy makers, health workers - they consider us criminals. They say 'there is no HIV in Egypt, we are a country of religion, of tradition, of good behaviour'. But they are in denial."
Slim, soft-spoken and admittedly shy, Magid (who declined to give his last name) is one of an estimated 34 million people living with HIV/AIDS across the world, with about 11,000 estimated cases in Egypt. Of those, the UN estimates only 400 are seeking treatment in Egypt.
June marks 30 years since the first reported case of HIV was diagnosed, but the joint United Nations programme on HIV/AIDS (UNAIDS) estimates that nine million people eligible for treatment still do not have access.
Officials say many other people infected with the virus do not even know they have it.
'Waiting to die'
Magid says he now knows how he got the virus, but at the time of his diagnosis the news came as a complete "shock".

"I went to submit my papers for military service and found out I had to go through a series of blood tests - one of them was for HIV," he says.
"But I wasn't told they'd be testing me for AIDS. The results came out and it turned out I was HIV positive."
Magid describes the time that followed as the "worst six months of my life".
"I spent my days in complete and utter shock. I could only think about what others would now think of me now that I had HIV.
"I didn't know anything about getting treatment or how I could live or what I could do or couldn't do because of HIV. I was just waiting to die. And I remained like this for half a year in complete denial - I couldn't come to terms with the unclear future that stood before me.
"I realised only after getting tested that I had taken the virus from someone else. Until now, I'm not sure if that person knows whether or not they are living with HIV because I haven't seen them since that day."
If he had not been tested by the military three years ago, Magid wonders if he would have ever known he had been infected. He says there is no excuse for forced testing, and says it is wrong for the military to make people undergo the tests without their knowledge or consent. And yet, Magid considers the experience a "God-send" - an opportunity that could have saved someone from catching the virus from him - and thus subsequently saving a life.
Months after his diagnosis, Magid joined Friends for Life, a non-governmental organisation in Egypt that works with people living with HIV/AIDS, where he says he was able to find "strength again", learn about living with the virus and meet others facing the same challenges.
Earlier this year, Magid addressed a session of the UN General Assembly in New York, becoming the first Arab to speak publicly about his struggles with the virus.
Amidst his advocacy work, Magid remains wistful, and says often he wishes he could return to a time when he was considered "normal". But these days, normal has a new definition.
Fastest-growing epidemics
Though he may be the first Egyptian to speak publicly about his HIV-positive status, Magid is certainly not alone, according to statistics provided in a new UN report.
Titled AIDS at 30: Nations at the Crossroads, the UNAIDS report says the Middle East and North Africa (MENA) region is one of two regions in the world with the fastest-growing AIDS epidemics.
Despite recent progress in getting the disease under control in many places around the world, UNAIDS says the MENA region has one of the highest rates of new infections, with an estimated 460,000 people currently living with HIV.
"We are a region that has our norms and culture and religion, so talking about sex and having an open dialogue is not that easy.
"[But] we are all interconnected. If we don't tackle the issue, it will come back to us"

That number is still relatively low compared to other regions, UN officials say, but it indicates a rise in the rate of new infections among the most at-risk members of the region's society.
The report places part of the blame for the rise of HIV/AIDS cases on stigma and discrimination that it says is prevalent across the region, imposing major obstacles to those seeking health services.
Lara Dabaghi, UNAIDS regional communications adviser for the Middle East and North Africa, says new infections also appear to be spreading due to low access to HIV prevention programmes for the most at-risk groups: sex workers, injecting drug users and men who participate in same-sex relations.
"These groups are highly stigmatised, so they don't seek treatment," she tells Al Jazeera from Cairo.
"Governments in the region do have treatment centres and they do offer treatment for free, but people do not access the treatment because of stigma and discrimination."
Punitive laws also hinder many from seeking the treatment they require, Dabaghi says, driving those living with HIV/AIDS underground, but failing to address the root causes of the epidemic.
"We are a region that has our norms and culture and religion, so talking about sex and having an open dialogue is not that easy. But if you don't address the topic and talk about it openly, [people infected with HIV] will hide," she says.
"We cannot reach them with prevention programmes. If I know I'm going to prison, there is no way I'm going to seek prevention services. So I become the 'hard-to-reach' population: males having sex with males who then get married because of social pressure, then infect their wives. Or drug users who then infect their wives, who then pass it on to their families.
"We are all interconnected. If we don't tackle the issue, it will come back to us."

Possible HIV prevention
Truvada is the trade name of a drug manufactured by the California-based company Gilead Sciences Inc which combines two antiretroviral drugs, used to treat Aids.
But this new study looks at whether it could be used to prevent HIV infection in the first place.

Almost 2,500 gay or bisexual men were randomly selected in Peru, Ecuador, Brazil, South Africa, Thailand and the United States. Half were given the pill, half were given dummy tablets.
All the men were also given condoms and counseling on safe sex.
What the researchers found after about a year of testing was that the drug appeared to cut male-to-male HIV transmission by 44%, when the group taking the pill was compared with the placebo group.
Those who took the pill regularly were deemed to have reduced their risk of infection further, by up to 73%, and blood tests were run to confirm this relationship between pill-usage and protection levels.
The research was funded by the Bill & Melinda Gates Foundation, and the federal US body, the National Institute of Allergy and Infectious Diseases (NIAID). The pills were donated by their manufacturer.
NIAID director, Dr Anthony Fauci, conceded more work needed to be done, but called the results impressive.
"This has been done in men who have sex with men. We need to know if we get similar results in women as well as in heterosexual men, which we have reason to believe we will," he told the BBC.
"We also need to get a long term view of were there any toxicities. We didn't see anything that was significant but we need to follow that for a long period of time."
Questions and concerns
The trial does of course raise questions and concerns. Is it possible, for instance, that the results were skewed by greater condom use in the group that took the pill; and won't such findings encourage some men to dispense with condoms altogether in favour of a drug?
There is also the issue of prohibitive cost of Truvada, which retails in the US for around $36 a day, and which makes the drug unaffordable to many possible users.
Dr Fauci argues that the two groups were fully randomised and says that drugs can only play a complementary role in the war on HIV. Condoms and fewer partners, he said, remain the first line of defence.
"We're hoping that if this does become a useable tool in prevention, then the associated counselling will complement the effect of the drug and stop people becoming cavalier about it and say 'now I have a pill I don't have to worry'.
"That's exactly the opposite of what we want to happen. We want to add something rather than have it replace something."
Sir Nick Partridge, chief executive of the Terrence Higgins Trust, called the trial results "potentially significant".
"It's vital that we expand the ways we can prevent HIV transmission, particularly amongst those most at risk," he said in a statement. "This trial proves that HIV treatment will have an impact on prevention, but that it's not ready for widespread use yet.
"Three major hurdles are still going to be its cost, the risks of drug-resistant strains of HIV developing and taking a drug treatment every day."

Bay Area officials optimistic about nation's first HIV/AIDS plan
The federal government released the first-ever national strategy Tuesday to combat the HIV/AIDS epidemic just as states and counties are shouldering deep cuts in HIV/AIDS prevention and drug programs.
The strategy focuses on reducing new HIV cases, increasing access to care and breaking down the stigma that persists among high-risk groups 30 years into the epidemic. Ultimately, the goal is to coordinate federal and state funds and spend them where they are most needed.
That comes at a crucial time for counties like Santa Clara, which saw its state funding for HIV prevention and care services drop last year by $1.2 million.
"I think "... this type of the strategy coming out of the presidential level gives new focus to an issue that has been really off the front pages," said Marty Fenstersheib, public health officer for Santa Clara County.

How Not to Raise a Bully: The Early Roots of Empathy
Since the Jan. 14 death of Phoebe Prince, the 15-year-old in South Hadley, Mass., who committed suicide after being bullied by fellow students, many onlookers have meditated on whether the circumstances that led to her after-school hanging might have been avoided.
Could teachers have stepped in and stopped the bullying? Could parents have done more to curtail bad behavior? Or could preventive measures have been started years ago, in early childhood, long before bullies emerged and started heaping abuse on their peers? Increasingly, neuroscientists, psychologists and educators believe that bullying and other kinds of violence can indeed be reduced by encouraging empathy at an early age. Over the past decade, research in empathy — the ability to put ourselves in another person's shoes — has suggested that it is key, if not the key, to all human social interaction and morality.
Without empathy, we would have no cohesive society, no trust and no reason not to murder, cheat, steal or lie. At best, we would act only out of self-interest; at worst, we would be a collection of sociopaths.
Although human nature has historically been seen as essentially selfish, recent science suggests that it is not. The capacity for empathy is believed to be innate in most humans, as well as some other species — chimps, for instance, will protest the unfair treatment of others, refusing to accept a treat they have rightfully earned if another chimp doing the same work fails to get the same reward.
The first stirrings of human empathy typically appear in babyhood: newborns cry when hearing another infant's cry, and studies have shown that children as young as 14 months offer unsolicited help to adults who appear to be struggling to reach something. Babies have also shown a distinct preference for adults who help rather than hinder others.
But like language, the development of this inherent tendency may be affected by early experience. As evidence, look no further than ancient Greece and the millennia-old child-rearing practices of Sparta and Athens. Spartans, who were celebrated almost exclusively as warriors, raised their ruling-class boys in an environment of uncompromising brutality — enlisting them in boot camp at age 7 and starving them to encourage enough deviousness and cunning to steal food, which skillfully bred yet more generations of ruthless killers.
In Athens, future leaders were brought up in a more nurturing and peaceful way, at home with their mothers and nurses, starting education in music and poetry at age 6. They became pioneers of democracy, art, theater and culture. "Just like we can train people to kill, the same is true with empathy. You can be taught to be a Spartan or an Athenian — and you can taught to be both," says Teny Gross, executive director of the outreach group Institute for the Study and Practice of Nonviolence in Providence, R.I., and a former sergeant in the Israeli army.
What the ancient Greeks intuited is supported by research today. Childhood — as early as infancy — is now known to be a critical time for the development of empathy. And although children can be astonishingly resilient, surviving and sometimes thriving despite abuse and neglect, studies show that those who experience such early trauma are at much greater risk of becoming aggressive or even psychopathic later on, bullying other children or being victimized by bullies themselves.
Simple neglect can be surprisingly damaging. In 2007, researchers published the first randomized, controlled study of the effect of being raised in an orphanage; that study, and subsequent research on the same sample of Romanian orphans, found that compared with babies placed with a foster family, those who were sent to institutions had lower IQs, slower physical growth, problems with human attachment and differences in functioning in brain areas related to emotional development.
Institutionalized infants do not experience being the center of a loving family's attention; instead, they are cared for by a rotating staff of workers, which is inherently neglectful. The infants miss out on intensive, one-on-one affection and attachment with a parental figure, which babies need at that vulnerable age. Without that experience, they learn early on that the world is a cold, insecure and untrustworthy place. Their emotional needs having gone unmet, they frequently have trouble understanding or appreciating the feelings of others. (Comment on this story.)
Nearly 90% of brain growth takes place in the first five years of life, and the minds of young children who have been neglected or traumatized often fail to make the connection between people and pleasure. That deficit can make it difficult for them to feel or demonstrate love later on. "You can enhance empathy by the way you treat children," says Martin Hoffman, an emeritus professor of psychology at New York University and a pioneer of empathy research, "or you can kill it by providing a harsh punitive environment."
