Chicago - Doctor gives risky drugs at high rate

November 10th -

Inside Chicago's Maxwell Manor nursing home, Dr. Michael Reinstein's patients suffered from side effects so severe that they trembled, hallucinated or lost control of their bladders.

Staffers told state investigators that so many patients were clamoring to complain to Reinstein about their medications that a security guard was assigned to accompany him on his visits. In addition, staffers said Reinstein had induced patients to take powerful antipsychotic drugs with the promise of passes to leave the home.

Though state officials shut that facility in 2000 for inadequate care and wretched conditions, Reinstein, the home's lead psychiatrist, continued to practice. Today he is one of the most prolific providers of psychiatric care in Chicago-area nursing homes and mental health facilities, even as he is trailed by lawsuits and complaints like the ones at Maxwell Manor.

Neither state nor federal officials appear to have ever assembled a complete picture of Reinstein's thriving practice, built in part within Illinois' poorly regulated system of nursing homes serving the mentally ill. But an investigation by ProPublica and the Tribune found that Reinstein has compiled a worrisome record, providing assembly-line care with a highly risky drug.

Searching publicly available documents, reporters discovered that Reinstein, 66, has been accused of overmedicating his mentally ill patients. His unusually heavy reliance on the drug clozapine -- a potent psychotropic medication that carries five "black box" warnings -- has been linked to at least three deaths.

In 2007 he prescribed various medications to 4,141 Medicaid patients, including more prescriptions for clozapine than were written by all the doctors in Texas put together, Medicaid records show.

Records also show he is getting government reimbursement for seeing an improbably large number of patients. Documents filled out by Reinstein suggest that if each of his patient visits lasts 10 minutes, he would have to work 21 hours a day, seven days a week. Reinstein sees 60 patients each day, he wrote in an audit report in 2007.

Controversial psychiatrist to head UM medical school department

Charles Nemeroff, an Atlanta psychiatrist who was the subject of a Senate investigation concerning huge sums he received from drug companies, is being named chair of the psychiatry department at the University of Miami medical school.
Last year Nemeroff, as chair of the Department of Psychiatry at Emory University, was the intense focus of an investigation by Sen. Charles Grassley, R-Iowa, who said he was concerned about the money the psychiatrist received from drug companies while conducting supposedly unbiased research for the National Institutes of Health on drugs made by the companies he was receiving money from.
On Thursday, Pascal Goldschmidt, dean of UM medical school, called Nemeroff ``an extraordinary psychiatrist and scientist. . . . He got into serious trouble on disclosure on conflict of interest.''
Goldschmidt said he had read investigative reports from Emory about Nemeroff's activities and found nothing to indicate that payments the psychiatrist received had in any way influenced his research results.
In a telephone interview at mid-day Thursday, Nemeroff, 60, told The Miami Herald he was excited to be coming to Miami. ``I think it's going to be a top-10 school.''
A front-page report by The New York Times in October 2008 said that congressional investigators found Nemeroff -- ``one of the nation's most influential psychiatrists'' -- had received $2.8 million in consulting deals with drug makers over seven years and failed to report at least $1.2 million of that to Emery University.
Based on Grassley's complaints, Emory suspended Nemeroff's work on an NIH grant and asked him to step down as chair of psychiatry while it studied his conduct. Earlier this year, The Atlanta Journal-Constitution reported that the Office of the Inspector General of the Department of Health and Human Services had launched an investigation into Nemeroff's activities.
The OIG said it never confirms nor denies any inquiries about investigations. Nemeroff said he knew nothing about an OIB investigation. NIH did not immediately respond on Thursday morning to Herald requests for comment. Nemeroff said he had been told by NIH that he could apply for grants as soon as he arrives in Miami.
About $800,000 to $1.2 million, according to published reports, came from GlaxoSmithKline, while Nemeroff was leading a major study into mood disorder drugs, including ones made by GSK.
Nemeroff said Thursday that the news reports had not made clear that the talks were on GSK drugs now on the market, while his research funded by NIH involved animal and lab studies of GSK chemical compounds that were years away from market.
The psychiatrist said in retrospect he should have declared the drug maker payments but that, at the time, he viewed the university standards as not requiring such revelations since the talks were of an educational nature. Emory has since changed its rules to make them more clear.
In a letter to Grassley last December, Emory officials wrote: ``We do not believe that Dr. Nemeroff's participation in the compensated speaking arrangements with GSK in any way biased the research conducted under the grant, although we will continue to ensure that no such bias existed.''
The Emory letter said Nemeroff's talks on behalf of GSK were ``focused on medical education and were not product specific or promotional. . . . As you alleged, Dr. Nemeroff did not disclose substantial speaking fees from pharmaceutical companies to Emory. Under federal regulations and Emory's policies, we believe he should have done so, although both the regulations and our policies could have been clearer.
``In Dr. Nemeroff's view, substantive, nonproduct specific talks focused on general medical education did not present a significant financial interest and were therefore not subject to disclosure under the United States Public Health Service.''

New Health Care Bill Drafted by House of Representatives Filled with Psycho/Pharma Industry Agenda

Information: The newly-forged 1990-page “Affordable Health Care for America Act” (HR 3962) offered by the House of Representatives is filled with mental health provisions intended to prop up psychiatry as well as the pharmaceutical industry with billions in future income. Key mental health components of this House bill:

MOTHERS Act
The bill includes the language of the MOTHERS Act, to “expand treatment for postpartum conditions” and calls for the development of “improved screening and diagnostic techniques,” but makes no provisions to ensure any entities doing such research are free from conflicts of interest or pharmaceutical funding. For example, Screening for Mental Health, Inc., and its sub-organization Signs of Suicide, who heavily promote and conduct mental health screening, received $4,985,925 from pharmaceutical companies prior to 2008, and ten leading psychiatric researchers have been exposed in the last year for failing to disclose millions of dollars in pharmaceutical payments. Despite their conflicts of interest and biased research, many pharma funded psychiatrists and researchers have been used by so-called advocacy groups (Screening for Mental Health, NAMI, etc which are also heavily funded by Pharma) to promote the need for federal laws that will only increase the number of Americans being needlessly targeted for psychiatric treatment and drugged. Yet this bill contains no provisions for full disclosure of conflicts of interest for any “entity” that could receive federal taxpayer funded grants, do research or promotional campaigns – such as the provision in the bill calling for a national PR campaign using TV, radio public and other public service announcements to urge women be screened and seek treatment for postpartum depression. The bill also calls for “clinical research” for the development of new treatments (drugs), but again, no guidelines for ensuring that any researchers/research entities are free from pharmaceutical funding or conflicts of interest. FBI Veteran Executive Calls For Special Counsel Investigation, Prosecutions in Sibel Edmonds Case

An 18-year Counterintelligence and Counterterrorism Manager for the FBI has called for a Special Counsel to be appointed to investigate the allegations of FBI translator-turned-whistleblower Sibel Edmonds. John M. Cole, who now works as an intelligence contractor for the Air Force, made his comments during an audio interview released late last week with radio journalist Peter B. Collins.

He also offered a detailed insider's look at the concerns among high-level officials inside the Bureau as Edmonds' disturbing allegations began coming to light back in 2002, before they would be quashed for seven long years by the Bush Administration's unprecedented use of the so-called "State Secrets Privilege" to gag her.

Earlier last week, following the publication of a remarkable American Conservative magazine cover story interview with Edmonds --- detailing a broad bribery, blackmail, and espionage conspiracy said to have been carried out between current and former members of the U.S. Congress, high-ranking State and Defense Department officials and covert operatives from Turkey and Israel, resulting in the theft and sale of nuclear weapons technology on the foreign black market --- Cole had been quoted by the magazine confirming one of Edmonds' key allegations.

"I am fully aware of the FBI's decade-long investigation of" Marc Grossman, he said in response to the AmCon article/interview. Grossman had served as the third-highest ranking official in the Bush State Department and was alleged by Edmonds in the interview, and in a sworn, video-taped deposition a month earlier, to have been the U.S. ringleader for a massive Turkish espionage scandal reaching through the halls of power and into top-secret nuclear facilities around the country to the benefit of allies and enemies alike. Cole said that the FBI's counterintelligence probe "ultimately was buried and covered up," and that he believes it is "long past time" for an investigation of the case to "bring about accountability."

In his subsequent interview with Collins last week (audio and text excerpts posted below) Cole elaborated on those comments in much greater detail, noting that Edmonds has been "one hundred percent right on the money, on the mark" and confirming the existence of an "ongoing and detailed effort by Turkey to develop influence in the United States" through various illegal activities.

"Yes, I can confirm that," Cole told Collins, "That's true."

The FBI veteran executive also offered an insider's account of the panic that ensued inside the highest echelons of the bureau following Edmonds' first disclosure of information in 2002, recounting how an executive assistant director admitted to him at the time, just after the story first broke, "Well, all I know is that everything that Sibel is stating is true. I read her file. Everything she stated is, in fact, accurate."
Cole further describes how the concerns about Edmonds ultimately led to the Bush Administration's two-time use of the Draconian "State Secrets Privilege" in hopes of keeping her extraordinary information from becoming public. "Everybody at headquarters level at the bureau knew that what she was saying was extremely accurate."

"I know they didn't want her to go out and speak about it at all," Cole revealed, "and I know they were trying to figure out ways of keeping this whole thing quiet, because they didn't want Sibel to come out."

He also offered information which directly counters one of the criticisms of Edmonds' allegations as frequently offered by skeptics. Namely, that as a short time FBI contract translator --- even though she was tasked to review some seven years of counterintelligence wiretaps made from 1996 to 2002 --- she couldn't have had enough understanding of the full scope of the investigations to understand what was really going on.

GIRALDI: What kind of information was Grossman giving to foreign countries? Did he give assistance to foreign individuals penetrating U.S. government labs and defense installations as has been reported? It’s also been reported that he was the conduit to a group of congressmen who become, in a sense, the targets to be recruited as “agents of influence.”

EDMONDS: Yes, that’s correct. Grossman assisted his Turkish and Israeli contacts directly, and he also facilitated access to members of Congress who might be inclined to help for reasons of their own or could be bribed into cooperation. The top person obtaining classified information was Congressman Tom Lantos. A Lantos associate, Alan Makovsky worked very closely with Dr. Sabri Sayari in Georgetown University, who is widely believed to be a Turkish spy. Lantos would give Makovsky highly classified policy-related documents obtained during defense briefings for passage to Israel because Makovsky was also working for the American Israel Public Affairs Committee (AIPAC).

GIRALDI: Makovsky is now working for the Washington Institute for Near Eastern Policy, a pro-Israeli think tank.

EDMONDS: Yes. Lantos was at the time probably the most outspoken supporter of Israel in Congress. AIPAC would take out the information from Lantos that was relevant to Israel, and they would give the rest of it to their Turkish associates. The Turks would go through the leftovers, take what they wanted, and then try to sell the rest. If there were something relevant to Pakistan, they would contact the ISI officer at the embassy and say, “We’ve got this and this, let’s sit down and talk.” And then they would sell it to the Pakistanis.
